Click on either of the links to download the nomination form for 2011. Material must have been published in 2010 or have a 2010 cover date. Only living persons may be nominated. All Fantasy is eligible: high fantasy, horror, sword & sorcery, supernatural, children's and YA books, and beyond. Comics are eligible in the Special Award Professional category. When listing stories or other material that may not be familiar to all the judges, please include pertinent information such as author, editor, publisher, magazine name and date, etc.

All ballots cast will be verified against membership lists from San Jose, Columbus, and San Diego.
